In consideration of being in VSP Mixed Adult League Association, hereby agree for myself, successors, heirs and assigns, release and forever discharge VSP Mixed Adult League Association, their membership, employees, officers and directors from all claims, actions or judgements I may have or claim to have against VSP Mixed Adult League Association for all personal injuries, including death and or pandemic/contagion diseases including Covid-19 but not limited to, plus injuries to property, real or personal caused by or arising out of my participation in VSP Mixed Adult League Association – either leagues or tournament.

I further agree for myself, successors, heirs and assigns to indemnity and hold VSP Mixed Adult League Association and its membership harmless from all claims and suits for personal injuries, including death, diseases (including pandemics/contagion), damages to property caused by my act of omission arising out of participation in VSP Mixed Adult League Association, and from all judgment recovered and from all expenses incurred in defending said claims. Any person(s) whom bring forth legal action against VSP Mixed Adult League Association will be subject to immediate removal from the Player Registration System (PRS). Removal from the PRS will make any and all ineligible to participate in any VSP Mixed Adult League Association Sanctioned event and an indefinite suspension will commence immediately. VSP Mixed Adult League Association reserves the right to share on a case-by-case basis all pertinent information with any and all sanctioning and or governing bodies.

I also acknowledge by participating in VSP Mixed Adult League Association there are inherent risks associated with playing this sport and willingly participate at my own risk.

I further agree that my photograph, pictures, slides or movies taken or made by VSP Mixed Adult League Association, their employees, officers and directors, in connection with my participation with VSP Mixed Adult League Association either leagues or tournaments or any reproduction of the same, as well as my name, may appear in any manner be used by VSP Mixed Adult League Association or by any person, corporation or association authorized by VSP Mixed Adult League Association. I am in good health and have no physical condition that would prevent me from participating in VSP Mixed Adult League Association events.

I have registered with VSP Mixed Adult League Association as a member and have personally entered my personal information as required pertaining to membership. I have read and understood the forgoing waiver and agree to the terms of participation.

I have read and completely understand all of the statements associated with this waiver. I also understand the inherent risks related to this activity. I also have had the opportunity to contact VSP Mixed Adult League Association (vanderhoofslopitch@gmail.com) and inquire about any of the terms in this waiver before agreeing to becoming a member of VSP Mixed Adult League Association.
